MLA format heading has become very commonly used in academic research hence its imperative that you must understand the right format which it's heading must take. The heading and the title of the research should only be included in the first page of the research and not in any other pages.

Your last name and the page number should also follow in that order. There should be no extra spaces between paragraphs; instead, it's a good if you can use the tab default settings to indent the paragraphs.
